This exciting study integrates genetics with gene expression regulation to uncover sex-specific differences in Crohn’s disease recurrence. Gene expression in 339 postoperative neoterminal ileum biopsies from Crohn’s disease patients were analyzed to understand why the disease often recurs after surgery. Genetic analyses revealed sex-specific effects and transcript changes related to lipid metabolism, membrane transport, and extracellular matrix processes. Overall, the findings highlight molecular mechanisms of postoperative recurrence that differ from those involved in initial disease development and point to new potential therapeutic targets beyond traditional cytokine-blocking treatments.

Last month, the IBDGC met for the annual IBDGC Face-to-Face Meeting at the Broad Institute, coinciding with ASHG 2025. The meeting consisted of presentations on progress, future goals, and great discussions, focusing around analyses and data projects. We even introduced a new segment to the meeting where we had analysts give flash presentations on current projects, with two top presenters selected by our External Advisory Board. Following the meeting, we gathered for dinner at Batifol Cambridge for some lovely French cuisine and drinks. Overall, the meeting was a great success! Big thanks to everyone who attended, and we also would like to thank the Broad Institute again for hosting and organizing this event. We look forward to the next time we get to meet face-to-face!

On October 14, 2025 the IBDGC will be convening for an annual face to face meeting. Taking advantage of the opportunity to attend one of the largest genetics conferences of the year, the IBDGC will utilize the first day of the conference as a springboard to evaluate aims and directions for the upcoming year. Representatives from all GRCs, the DCC, NIDDK and our newly created EAB will be attending this meeting.

Over the past two decades, significant progress has been made in understanding the genetic factors behind inflammatory bowel disease (IBD), laying the groundwork for genomic medicine. The NIDDK IBD Genetics Consortium identifies 11 key challenges for the next two decades, focusing on genetic architecture, cellular and molecular characterization, and personalized medicine applications. These challenges include investigating genetic variations across populations, refining causal variants, and developing precision therapeutics to address unmet medical needs.

Building on top of existing data available from the Ileal Resection cohort, the focus of this paper was tissue-associated microbiota. With 1,100+ samples obtained from three clinical time points, Cristian Hernenadez-Rocha, MD found distinct microbiome signatures that could predict future disease recurrence.

The IBDGC gathered in Washington, DC for a mid-year meeting to discuss ongoing progress in the working groups and to welcome newly awarded Ancillary R01 awardees into the consortium. Another major discussion point included leveraging clinical collaborators to increase recruitment to the Diversity in IBD study.

Cristian Hernandez-Rocha, MD along with Mark Lazarev, MD led the analysis using the Crohn's Disease Ileal Resection longitudinal study. Relying on data obtained from patients recruited to the study and their medical records, a multitude of clinical variables were assessed to determine which has the highest impact on disease recurrence. Several independent clinical predictors were identified as potentially important indicators that are not currently used in clinical practice.

On November 1st, the IBDGC held its annual in-person meeting in Washington, DC. The working group presentations led to very fruitful discussions that will undoubtedly help guide the future direction of the IBDGC's research and analysis in the coming months. High engagement from both trainees and senior investigators made for a synergistic meeting that marks one of the most successful in the IBDGC's history.

NIDDK will be funding up to seven R01 awards with funding beginning in December 2023 or July 2024. A letter of support from the IBDGC Steering Committee is required. Investigators must submit a request for collaboration to the IBDGC by September 08, 2023, for the November 08, 2023 due date.
